for Cardiac MRI DRAFT LCD RAD-024
Request that the Cardiac MRI stress series of codes (75559, 75563) be moved to the
cardiac perfusion LCD CV-017. Currently the indications for a stress MRI
are not addressed in either the new draft or prior LCD RAD-024. Do we need to obtain an ABN every time and inform the patient that Medicare does not cover this? Directive in the LCD would be appreciated.

Response:
CPT codes 75558, 75560, 75562 and 75564 are deemed non-payable by Medicare and have been removed from this LCD.
If a beneficiary wishes a non-covered service to be performed, no ABN is needed. However, the decision whether to perform the service and/or to bill for it is not a Medicare issue.

from WPS received requests for the addition to RAD-024 of multiple ICD-9 codes

Response:
WPS has evaluated requests for inclusion of multiple ICD-9-CM codes and determined the following codes valid for inclusion in RAD-024;
MRI Head 70336, 70540, 70542, 70543, 70551, 70552:
ICD-9 code 255.0 will be added to RAD-024.
MRI Cervical Spine
ICD-9 code 781.2 will be added to RAD-024
MRI Cardiac 75557, 75559, 75561, 75563:
ICD-9 codes 429.3 and 441.2 will be added to RAD-024
MRI Lower Extremity (73718-73723)
ICD-9 codes 094.0, 713.5 and 781.2 will be added to RAD-024

Comments:
A request was received to add to RAD -024 the ICD-9 codes for diabetes mellitus. (250.00-250.93).

Response:
Although diabetes can lead to medical problems that may require an MRI, diabetes by itself is not a primary indication for this diagnostic imaging service.

Comments:
A comment was received concerning the performance of a MRI procedure during a viable pregnancy or on a patient that has a pacemaker

Response:
The decision to perform MRIs during pregnancy or for a patient that has a pacemaker is a decision best left to the physicians involved in the patients' care.
